Paxil (paroxetine HCl) is a member of a class of prescription drugs called selective serotonin reuptake inhibitors (SSRIs), and serves to relieve depression. Paxil has been approved for the treatment of major depressive disorder, generalized anxiety disorder (GAD), social anxiety disorder (SAD), panic disorder, obsessive-compulsive disorder (OCD) and posttraumatic stress disorder (PTSD). With continued treatment, Paxil can help restore the balance of serotonin (a naturally occurring chemical in the brain) -- which helps reduce the symptoms of anxiety and depression. Paxil is available in tablet and liquid form.

How To Take Paxil CR and Paxil CR Dosage and Administration

Paxil comes in 10mg, 20 mg, 30 mg, and 40 mg tablets. The usual startup dose is one 10 mg or one 20 mg tablet a day. For people over the age of 65 the initial startup dose is 10 mg a day. For best results Paxil should be taken at the same time every day, preferably in the morning.

Paxil CR Side Effects

The most common side effect is nausea, which can be alleviated by taking Paxil with food. Other side effects that may occur include sweating, decreased appetite, asthenia (lack of strength), dizziness, tremor, insomnia, drowsiness, nervousness, and/or decreased sexual drive. Side effects of Paxil are generally mild and temporary.

There have been indications that stopping this drug can cause quite distressing physical and psychological symptoms. According to World Health Organization data, Paxil has the highest incidence rate of withdrawal adverse of any antidepressant in the world.

It is now generally recognized that SSRIs can cause a condition called akathisia (severe inner restlessness) that in turn is associated with suicidality. In the most recent edition of Diagnostic and Statistical Manual of Mental Disorders, Fourth Edition, Text Revision ("DSM IV TR") under Differential Diagnosis it states: "Serotonin-specific reuptake inhibitor antidepressant medications may produce akathisia... Akathisia may be associated with dysphoria (restlessness, depression & anxiety), irritability, aggression, or suicide attempts."

Suicidality associated with Paxil can be the result of recent ingestion (particularly during the first few weeks of treatment initiation or dosage increase) or trying to stop the drug. There are multiple reports of suicides occurring when people are trying to get off of the drug.

It has been estimated that as many as 50% to 75% of the people taking Paxil experience dependency/withdrawal syndrome. The physical and psychological effects include the following:

Electrical 'zaps' to the brain, dizziness, extreme nausea, crying spells, light headedness, vertigo, coordination problems, gait disturbances, sweating, vomiting, high fever, abdominal discomfort, flu symptoms, agitation, tremulousness, irritability, aggression, sleep disturbance, nightmares, tremor, confusion, memory and concentration difficulties, lethargy, malaise, weakness, and general fatigue.

However, Glaxo Smith Kline, the manufacturer of Paxil maintains that Paxil is a safe and effective drug.

Paxil CR Precautions and Contraindications

Paxil is not intended for everyone. To avoid serious complications, you may want to talk to your doctor if you are pregnant or nursing, have kidney or liver disease, or suffer from seizures.

Paxil CR Drug Interactions

It's very important that your doctor know about any prescription and/or herbal products you are taking. Paxil is not intended for patients taking MAO Inhibitors (older forms of antidepressants). If you are taking MAO Inhibitors consult with your doctor before you begin treatment. Serious, and sometimes fatal complications have occurred when these medicines have been used together.

Additionally, do not take Paxil if you are taking thiorizaine (Mellaril). The combination can cause dangerous even fatal irregular heartbeats.

Paxil CR Lawsuit and Litigation

Paxil Follows Prozac Into Courtroom

The maker of Paxil will be in a Wyoming courtroom next week to defend its antidepressant against charges that the drug caused a user to shoot three family members and himself to death.

GlaxoSmithKline faces the same charges that Eli Lilly and Co. has beaten twice in court over the alleged ability of its drug Prozac to induce suicide and violence.

Set to start Monday in U.S. District Court in Wyoming, the jury trial will focus on whether Paxil triggered Donald J. Schell to kill his wife, Rita, daughter Deborah M. Schell Tobin, granddaughter Alyssa Ann Tobin, and himself in February 1998.

Schell had been prescribed Paxil for depression two days before the shootings, according to the lawsuit brought by two surviving family members.

The wrongful-death lawsuit says British-owned GlaxoSmithKline, one of the world's largest drug companies, was negligent and reckless for not warning doctors and patients that Paxil can induce violence.

Houston trial lawyer Andy Vickery, who tried and lost a similar lawsuit against Lilly over Prozac, represents the plaintiffs in the Wyoming case.

He won a key pretrial battle earlier this month when U.S. Magistrate Judge William C. Beaman rejected GlaxoSmithKline's motion to exclude or limit testimony from the plaintiffs' expert witnesses.

The plaintiffs have lined up Wales psychiatrist Dr. David Healy and Dr. John T. Maltsburger, an associate clinical professor of psychiatry at Harvard Medical School, to testify that Paxil may have triggered Schell's deadly rampage.

GlaxoSmithKline's experts include Dr. J. John Mann, a leading suicidologist who is a professor of psychiatry at Columbia University.

The makers of the Prozac-class of antidepressants, which includes Pfizer Inc.'s Zoloft, have never lost a civil wrongful-death court case against their popular drugs, though hundreds of lawsuits have been filed.

A victory by plaintiffs in the case could prompt more such lawsuits against the major antidepressant makers or force them to pay more to settle pending cases.

The Food and Drug Administration also could face pressure to require the antidepressant manufacturers to put warnings about suicide and violence on their drugs' labels.
